Responsibilities

The Licensed Practical Nurse (LPN) in Home Health provides quality patient care and performs technical skilled care in the patient's home. Services are provided within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team. The LPN may not alter the plan of care. The LPN instructs the patient's family in care and maintaining a healthy environment. Observes, records, and reports the patient's response to treatment and any changes in condition to the supervising nurse or physician.

Education and Experience

License Requirements:

• Current LPN licensure in the state of practice
• Valid Driver's License, vehicle insurance, and access to reliable transportation
• CPR Certification
