Medina Center for Rehabilitation seeks skilled LPNs to join our compassionate team in Medina, OH. The role involves administering medications, monitoring residents, and participating in individualized care planning within a supportive, patient-centered environment.

We offer part-time day shifts with weekend rotation, shift differentials, benefits, and ongoing training. LPNs must hold a current Ohio license and be prepared for 12-hour shifts in a rehabilitation and long-term care setting.

Qualifications

• Current Ohio LPN license is required.
• Active license in good standing with the state board is expected.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor and document residents' health, vital signs, and responses to therapies.
• Assist in developing and implementing individualized care plans.
• Provide nursing care including wound, catheter care, and activities of daily living.
• Collaborate with physicians, RNs, and healthcare team for coordinated care.
• Educate residents and families on health management and procedures.
• Ensure care aligns with regulatory and facility guidelines.
• Respond to emergencies and provide critical care when needed.
• Assist with training and supervision of CNAs.

• Current Licensed Practical Nurse (LPN) Ohio license

Responsibilities of Licensed Practical Nurses (LPN):
