Job summary

Hampton Woods Nursing Center is seeking an LPN to provide skilled nursing care in our facility. You will administer medications, monitor patients, and support IV and tube feeding as needed.

Shifts are 12 hours, 7p–7:30a, with a 3-day workweek and every other weekend off. Ohio-licensed LPNs with LTC experience are preferred and a pre-employment background check is required.

Qualifications

• Must hold an LPN license; Ohio license required.
• 1 year LTC/Rehab experience preferred.
• Prior IV insertion and tube-feeding skills preferred.

Responsibilities

• Administer medications and treatments per protocols.
• Provide direct patient care including IV and tube feeding as needed.
• Assess patients and monitor vital signs for health status.
• Maintain accurate EHRs and HIPAA compliance.
• Collaborate with interdisciplinary teams on care plans.
• Educate patients and families on health management.

Schedule & Eligibility

• 12 Hour Shifts - 7p-7:30a; Night Shift
• Work 3 Days a week to be considered full-time and receive full-time benefits!
• Every Other Weekend scheduled off
• Applicants must be an LPN, licensed in Ohio and pass a pre-employment background check.

Responsibilities

• Administer medications and treatments, including IV insertion and tube feeding, while ensuring adherence to established protocols.
• Provide direct patient care, including catheterization, tube feeding, and IV infusion as needed.
• Conduct patient assessments and monitor vital signs to ensure optimal health.
• Maintain and utilize accurate electronic health records (EHR) and ensure compliance with HIPAA regulations.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans.
• Educate patients and families on health management and wellness strategies.
