Job summary

Medina Center for Rehabilitation and Nursing is hiring Licensed Practical Nurses (LPNs) for all shifts on a part-time and PRN basis. The role includes 12-hour shifts, weekend availability, and shift differentials in a rehabilitation and long-term care setting.

Qualifications

• Current LPN Ohio license is required.

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Monitor and document residents' health conditions, including vital signs, symptoms, and responses to treatments.
• Assist in the development and implementation of individualized care plans.
• Provide nursing care for residents, including wound care, catheter care, and assistance with daily living activities.
• Collaborate with physicians, RNs, and other healthcare team members to ensure coordinated and comprehensive care.
• Educate residents and their families about health management and procedures.
• Ensure that all care is delivered in accordance with regulatory and facility guidelines.
• Respond to emergencies and provide critical care when necessary.
• Assist with training and supervising nursing assistants (CNA).

Education
