Community Skilled Health Care Centre in Ohio is hiring Licensed Practical Nurses (LPNs) for night shift. We offer Full-Time and PRN Nights with 12-hour shift options, every other weekend rotation, and PRN flexibility.

The role emphasizes patient-centered care within a supportive, team-driven environment. Responsibilities include administering medications, monitoring residents, updating care plans, wound care, and collaborating with RNs and physicians.

Qualifications

• Active Ohio LPN license.
• Long-term care experience preferred, but not required.
• Strong clinical and communication skills.
• Ability to work independently on night shift.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor and document residents’ conditions, including vital signs and changes in status.
• Assist in developing and updating individualized care plans.
• Provide direct nursing care including wound care and daily living support.
• Collaborate with RNs, physicians, and interdisciplinary team members.
• Ensure compliance with all facility and regulatory standards.
• Respond to emergencies and provide appropriate care.
• Supervise and support nursing assistants (STNAs/CNAs).
