Job summary

The Grand Rehabilitation and Nursing at Queens in New York is seeking a full-time Licensed Practical Nurse (LPN) to join our skilled nursing facility team. The ideal candidate has a NY LPN license, a passion for geriatric care, and the ability to evaluate and treat residents independently while coordinating with physicians, NPs, families and the care team.

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit. Duties Include;
• Medication Administration, including IVs.
• Taking and recording vital signs.
• Providing resident and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
• Supervising CNAs
