Job summary

Greenhills Health and Rehabilitation Center in Kingston Springs, Tennessee is seeking an Occupational Therapist to join our team. The therapist will assess referred patients, develop and implement individualized treatment plans, and provide direct patient care within our rehabilitation program.

The role requires supervision of Occupational Therapy Assistants, collaboration with interdisciplinary teams, and thorough documentation of patient progress while educating patients and families.

Qualifications

• Graduate from an accredited OT program.
• Must hold a valid state OT license/registration.
• Experience in long-term care or geriatric settings preferred.
• Strong documentation and record-keeping skills.
• Excellent patient-service orientation and professionalism.

Responsibilities

• Assess referred patients to determine rehabilitation needs and potential for recovery.
• Provide direct patient care and implement treatment programs with goals set by the physician.
• Document patient status, progress, and discharge plans; educate patients and families.
• Oversee and supervise Occupational Therapy Assistants.
• Collaborate with all disciplines to provide patient-centered care.
• Prepare weekly notes, re-certifications, and discharge documentation.
• Ensure confidentiality and compliance with HIPPA and residents’ rights.

Occupational Therapist

Greenhills Health and Rehabilitation Center is looking for a talented and hard-working OccupationalTherapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patients condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
