Job summary

Greenhills Health and Rehabilitation Center is seeking an Occupational Therapist to join our team. The OT will assess referred patients requiring rehabilitation, provide direct patient care, and develop treatment programs tailored to each patient’s capacity and tolerance under the direction of the Physician.

The role demands setting realistic goals, documenting patient condition and progress, educating patients and families about therapy methods, and supervising Occupational Therapy Assistants.

Qualifications

• Graduated from an accredited OT program and licensed for the state.
• Experience with long-term/geriatric care preferred.
• Ability to supervise Occupational Therapy Assistants.
• Strong documentation and record keeping.
• Collaborates with other disciplines to plan patient-centered care.

Responsibilities

• Assess referred patients needing rehabilitation and provide direct patient care.
• Develop and administer treatment plans with goals based on patient capacity.
• Document patient condition and progress, including evaluations and discharge docs.
• Educate patients and families on therapy approaches.
• Supervise Occupational Therapy Assistants.
• Coordinate with other disciplines to deliver patient-centered care.
• Ensure confidentiality and HIPAA compliance.
• Meet scheduled hours and maintain professional standards.

Occupational Therapist

Greenhills Health and Rehabilitation Center is looking for a talented and hard-working OccupationalTherapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patients condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
