Job summary

Greenhills Health and Rehabilitation Center is seeking a skilled Occupational Therapist to join our rehabilitation team. The OT will assess referred patients, develop and implement treatment plans, and set achievable goals based on each patient’s capacity.

You will educate patients and families and supervise OT Assistants as part of a collaborative, interdisciplinary team. The role emphasizes high-quality patient care in a long-term care setting, with strict adherence to documentation standards

Qualifications

• Graduated from an accredited Occupational Therapy program.
• Must hold a valid OT license and registration for the state of practice.
• Experience supervising Occupational Therapy Assistants.
• Experience in long-term care or geriatric settings preferred.
• Strong documentation and record-keeping skills.
• Experience with evaluations, progress notes, re-certification and discharge documentation.
• Collaborates with other disciplines to provide patient-centered care.
• Demonstrates professionalism and exceptional patient service.
• Complies with confidentiality and residents' rights regulations.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Develop and administer an OT treatment program with goals based on patient capacity.
• Set realistic and achievable goals and document patient progress.
• Educate patients and families in appropriate therapy methods.
• Oversee and supervise OT Assistants and coordinate with the rehab team.

Occupational Therapist

Greenhills Health and Rehabilitation Center is looking for a talented and hard-working OccupationalTherapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patients condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
