Job summary

Wexford House in Kingsport, Tennessee is seeking compassionate LPNs to join our care team. You'll play a key role in residents' daily care and coordinate with CNAs to deliver high-quality nursing services.

As an LPN, you will assess residents, administer medications per physician orders, help with admissions and transfers, and contribute to care plans while maintaining confidentiality. We offer benefits and a supportive team.

Qualifications

• Current unrestricted LPN License for Tennessee.
• CPR/BLS certification.

Responsibilities

• Direct the daily activities of the CNAs and ensure policies and procedures are followed by nursing personnel.
• Participate in admissions, discharges and transfers of residents as required.
• Assess resident’s conditions, prepare, and administer medications per physician orders.
• Initiate, review, and update care plans.
• Report any changes in a resident’s condition to the nurse supervisor and keep them informed of factors that interfere with being able to perform work as assigned.
• Perform other assigned duties given by clinical or administrative leadership.
• Maintain confidentiality of protected health information, including verbal, written and electronic communications.

LPN Position Summary
