Human Touch Home Care in Colorado Springs is hiring a compassionate Licensed Practical Nurse (LPN) to provide in-home nursing care under RN supervision. The role includes assessing patients, implementing plans of care, and educating families to support self-care.

Pay is per visit, not per hour, with a full-time schedule and benefits. Requirements: Colorado LPN license, CPR, and 1 year in home health preferred. In-person work with weekly schedule Monday through Friday.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three (3) years nursing experience; home health or medical/surgical preferred.
• Active Colorado LPN license.
• CPR certification.

Responsibilities

• Provides direct patient care as defined in the state Nurse Practice Act.
• Implements plan of care initiated by the RN.
• Documents accurately and in a timely manner per the plan of care.
• Educates patient and family on self-care and diagnosis-related information.
• Coordinates home health services with other disciplines and reports needs to RN/supervisor.
• Uses and maintains equipment per aseptic technique.
• Participates in ongoing professional growth and development.
• Performs other duties as assigned by the RN.

Human Touch Home Care is seeking a compassionate and dedicated Home Healthcare License Partitioner Nurse (LPN). Our Home Health LPNs deliver high-quality nursing care to patients in their homes under the supervision of a registered nurse RN. This role involves assessing patient needs, implementing care plans, and assisting with daily activities to enhance patient well-being.

Responsibilities:

• Provides direct patient care as defined in the state Nurse Practice Act.
• Implements plan of care initiated by the registered nurse.
• Provides accurate and timely documentation consistent with the plan of care.
• Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
• Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietitian) to the registered nurse and/or clinical supervisor.
• Uses and/or prepares equipment and supplies effectively and efficiently adhering to aseptic technique.
• Participates in personal and professional growth and development.
• Performs other duties as assigned by the registered nurse.

Qualifications:

• Graduate of an accredited practical nurse or vocational nursing program.
• Has three (3) years nursing experience. Community/home health or medical/surgical experience is preferred.
• Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
• Possess and maintain valid CPR certification.
• Complies with accepted professional standards and practice.
• Demonstrates good verbal and written communication and organizational skills.

$30.00 - $45.00 per visit
