Job summary

Human Touch Home Health Care in Colorado is seeking a compassionate LPN to deliver in-home nursing care under RN supervision. You will assess patient needs, implement care plans, and assist with daily activities to improve well-being.

The role is PRN with a strong emphasis on patient education, documentation, and coordination with the care team to ensure high-quality home health services for clients in the community.

Qualifications

• Graduate of an accredited LPN/LVN program.
• Currently licensed as an LPN/LVN in Colorado.
• CPR certification required.
• 3 years nursing experience; home health preferred.

Responsibilities

• Provide direct patient care per the Nurse Practice Act.
• Implement the plan of care initiated by the RN.

Human Touch Home Care is seeking a compassionate and dedicated Home Healthcare License Partitioner Nurse (LPN). Our Home Health LPNs deliver high-quality nursing care to patients in their homes under the supervision of a registered nurse RN. This role involves assessing patient needs, implementing care plans, and assisting with daily activities to enhance patient well-being.

Responsibilities

• Provides direct patient care as defined in the state Nurse Practice Act.
• Implements plan of care initiated by the registered nurse.
• Provides accurate and timely documentation consistent with the plan of care.
• Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
• Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietitian) to the registered nurse and/or clinical supervisor.
• Uses and/or prepares equipment and supplies effectively and efficiently adhering to aseptic technique.
• Participates in personal and professional growth and development.
• Performs other duties as assigned by the registered nurse.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Has three (3) years nursing experience. Community/home health or medical/surgical experience is preferred.
• Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
• Possess and maintain valid CPR certification.
• Complies with accepted professional standards and practice.
• Demonstrates good verbal and written communication and organizational skills.
