Human Touch Home Health Care is seeking Licensed Practical/Vocational Nurses (LPN/LVN) to provide direct patient care in Colorado Springs under the supervision of a registered nurse. Responsibilities include following the plan of care, delivering treatments, and collaborating with the care team to achieve positive patient outcomes.

The role is full-time and compensates per visit rather than hourly, with a typical visit lasting about one hour.

Qualifications

  • Graduate of an accredited practical nurse or vocational nursing program.
  • Three years nursing experience; home health or medical/surgical preferred.
  • Currently licensed as LPN/LVN in the state.
  • Valid CPR certification.
  • Has a driver's license and reliable vehicle.

Responsibilities

  • Provides direct patient care as defined in the state Nurse Practice Act.
  • Implements plan of care initiated by the registered nurse.
  • Documents accurately and timely consistent with the plan of care.
  • Educates patient and family on self-care and diagnosis-related info.
  • Coordinates home health services with other disciplines.
  • Uses equipment per aseptic technique and maintains supplies.
  • Engages in ongoing personal and professional development.
  • Performs other duties as assigned by the RN.

Human Touch Home Care is hiring Licensed Practical Vocational Nurses in the Colorado area. The Licensed Practical Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities including following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

Responsibilities
  • Provides direct patient care as defined in the state Nurse Practice Act.
  • Implements plan of care initiated by the registered nurse.
  • Provides accurate and timely documentation consistent with the plan of care.
  • Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
  • Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietitian) to the registered nurse and/or clinical supervisor.
  • Uses and/or prepares equipment and supplies effectively and efficiently adhering to aseptic technique.
  • Participates in personal and professional growth and development.
  • Performs other duties as assigned by the registered nurse.

Qualifications
  • Graduate of an accredited practical nurse or vocational nursing program.
  • Has three (3) years nursing experience. Community/home health or medical/surgical experience is preferred.
  • Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
  • Possess and maintain valid CPR certification.
  • Complies with accepted professional standards and practice.
  • Demonstrates good verbal and written communication and organizational skills.
  • Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.
