Aventura at Pembrooke in West Chester, PA is hiring Licensed Practical Nurses (LPNs) to join our compassionate team. You will provide direct nursing care to residents, work with a collaborative care team, and help ensure the highest quality of care in a safe, supportive environment.

Shifts include 7a-3p, 3p-11p, and 11p-7a with FT/PT options. We offer competitive pay ($28-$33/hr depending on experience and shift), daily pay, full benefits for FT, career growth opportunities, and a friendly

Qualifications

• Valid Licensed Practical Nurse (LPN) license in PA.
• At least 6 months of experience in a skilled nursing or healthcare setting.
• CPR certification required.
• Compassionate, patient-focused and committed to high-quality care.
• Ability to work independently and in a team.
• Excellent communication and interpersonal skills.
• Ability to maintain accurate records and handle sensitive information.

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Monitor and document residents' health conditions, including vital signs and responses to treatments.
• Assist in development and implementation of individualized care plans.
• Provide nursing care including wound, catheter care, and daily living assistance.
• Collaborate with physicians, RNs, and the health-care team for coordinated care.
• Educate residents and families about health management and procedures.
• Ensure care aligns with regulatory and facility guidelines.
• Respond to emergencies and provide critical care when necessary.
• Assist with training and supervising CNAs.

Qualifications for Licensed Practical Nurses (LPN):

• Valid Licensed Practical Nurse (LPN) license in PA
• Previous experience in a skilled nursing facility, rehabilitation center, long-term care, or healthcare setting for at least 6 months.
• CPR Certification
• Compassionate, patient-focused, and committed to providing high-quality care.
• Ability to work independently and collaboratively within a team.
• Excellent communication and interpersonal skills.
• Ability to maintain accurate records and handle sensitive information confidentially.

Pre-Hire Requirements

• Current physical examination completed within the past 2 years
• Completed PPD/TB test
• Current CPR certification required
• Completed background check
• Current nursing license
