Complete Care at Oak Ridge in Charleston, WV is hiring an LPN licensed practical nurse to provide direct care for residents recently discharged from hospital or in long-term care. You will administer medications, monitor health status, and collaborate with a care team to implement individualized plans.

Qualifications

• Licensed to practice as an LPN in West Virginia.
• Able to pass background check, drug screen, and references.
• Professional and clinically skilled with leadership qualities.

Responsibilities

• Administer medications: oral, topical, IV, injectable.
• Wound care: perform wound care, manage pain, apply dressings.
• Monitor health status: observe physical and mental health, track changes and analyze data.
• Clinical procedures: catheter/ostomy care, specimen collection.
• Care planning: assist in executing individualized care plans.
• Collaboration: work with physicians, therapists, social workers and others.
• Family and resident education: explain health status and care expectations.
• Documentation: maintain detailed health records and progress notes.
• Team oversight: supervise and delegate to CNAs to ensure quality care.

Here’s What You’ll Do for Our Residents:
