Job summary

Rockville Center is seeking a Licensed Practical Nurse (LPN) to join our Skilled Nursing team. The role focuses on geriatric patient care in a SNF/Long Term Care unit, with licensed practice in NY and the ability to work independently with supervision as needed.

Key duties include medication administration (including IVs), vital signs, therapies, and resident/family education, plus coordinating with physicians and supervising CNAs. Weekend shifts alternate.

Qualifications

• Current NY LPN license.
• Alternate weekends required.

Responsibilities

The position is within a Skilled Nursing facility. Candidate should possess passion for geriatric population, must contain a license in the state of NY, be able to evaluate and treat independently without direct supervision, and posses the desire to perform functional therapeutic activities to assist residents improve in their experience.

LPN (Licensed Practical Nurse) Job Description:

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit. Duties Include;
• Medication Administration, including IVs.
• Taking and recording vital signs.
• Providing resident care and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
• Supervising CNAs
