The Elms Rehabilitation and Skilled Nursing in Milford, NH seeks an LPN to provide direct nursing care and supervise nursing assistants, coordinating day-to-day tasks across the care team.

You will monitor residents, administer medications, perform treatments, and document physician orders, ensuring safe, responsive care and strong family/provider communication in a collaborative setting.

Qualifications

• Graduate of an accredited LPN/LVN program.
• Current unrestricted LPN license in the practicing state.
• Current CPR certification.
• Strong clinical judgment, communication, and leadership skills.
• Ability to organize priorities and stay composed in a changing healthcare environment.
• Prior long-term care experience preferred.

Responsabilités

• Provide direct nursing care and supervise assigned nursing assistants and support staff.
• Monitor resident status and promptly communicate significant changes.
• Administer medications and monitor residents for adverse reactions.
• Perform wound care, treatments, and nursing procedures within scope of practice.
• Accurately carry out and document physician orders.
• Participate in admissions, discharges, transfers, rounds, and care planning.
• Coordinate follow-up appointments and transportation needs.
• Collaborate with residents, families, providers, and the interdisciplinary team.
• Support end-of-life care when needed.
• Promote teamwork, regulatory compliance, and survey readiness.
