Job summary

The Moments of Lakeville is seeking compassionate LPNs to join our multi-nursing team on day, evening, NOC and weekend shifts in Lakeville, MN. You will ensure resident safety, lead a care team, and collaborate with physicians and families to craft person-centered plans of care.

Qualified candidates are licensed LPNs in Minnesota, with strong communication and prior EMR experience; BLS preferred. This role offers a stable, caring environment with 24/7 on-site nursing coverage.

Qualifications

• Must be licensed by Minnesota as an LPN and maintain current licensure.
• Experience with EHR/EMR systems, preferably PointClick Care.
• Strong communication with residents, families, physicians and staff.

Responsibilities

• Ensure health and safety of all residents.
• Lead and direct care team; perform resident cares as needed.
• Collaborate to develop person-centered plans of care.
