Job summary

The Moments of Lakeville, MN is seeking skilled LPNs to join our compassionate memory care and enhanced assisted living team on the Lakeville Campus. Evening and weekend shifts are available, part-time and full-time.

Responsibilities include leading care teams, ensuring resident safety, and developing person-centered plans. Minnesota LPN license and TB background check required; BLS preferred. Competitive pay starts around $30/hour with shift differentials.

Qualifications

• Must be licensed with the State of Minnesota as an LPN and maintain current licensure.
• Experience with EHR/EMR systems, ideally Point Click Care.
• Ability to work with a team to develop person-centered care.
• Excellent judgment and decision-making abilities.
• Background check and TB screening required.

Responsibilities

• Ensure the health and safety of all residents.
• Lead and direct your care team along with actively performing cares of each resident as needed.
• Collaborate with residents, families, physicians and Moments staff to create a person-centered plan of care.
• Support orientation and education requirements of staff.
• Develop person-directed care and programming for residents with dementia.
