Job summary

Logan Health in Shelby, Montana seeks an LPN for an Intermediate Swing Bed role. You will provide nursing care to intermediate swing bed patients, coordinate with CNAs, and collaborate with acute care staff to ensure steady, compassionate care.

The role emphasizes patient relationships, continuity of care, and a calm, supportive environment. Ideal candidates have Montana LPN licensure, BLS, and 2+ years of nursing experience, with long-term care or swing bed experience a plus.

Qualifications

• Graduate of an accredited RN program
• Current Montana LPN license or multistate compact license
• BLS required
• ACLS, PALS, or NRP preferred
• At least 2 years of nursing experience preferred
• Experience with long-term care, rehab, or swing bed patients is a plus

Responsibilities

• Provide nursing care to intermediate swing bed patients
• Support patients who have completed or no longer qualify for skilled therapy
• Communicate clearly and compassionately with patients and their families
• Lead and support CNAs and other care partners on the unit
• Collaborate with acute care nursing staff when questions or needs arise
• Maintain a calm, steady approach to patient care in a lower-acuity setting

Education

Montana LPN license

BLS certification

ACLS/PALS/NRP preferred

2+ years nursing experience

Experience with long-term care/swing bed
