Care IV Home Health in Little Rock, AR is hiring an LPN Intake Coordinator for a full-time, office-based role at our corporate headquarters. You will manage intake calls, describe service options, and verify payer networks, while supporting the Intake Manager with orders and documentation.

Ideal candidates hold an active LPN license in good standing, have recent clinic experience, strong computer skills, and knowledge of nursing processes.

Qualifications

• Active LPN license in good standing with the Arkansas State Board.
• Recent clinic or medical office experience.
• Strong computer skills and familiarity with office equipment.
• Solid clinical nursing knowledge with skilled nursing background.
• Home Health experience preferred but not required.

Responsibilities

• Handle calls from prospective patients and referral sources.
• Describe Care IV's home health service options to referrals and patients.
• Assess patient suitability for home health care over the phone.
• Verify payer source is in-network with Care IV.
• Support the Intake Manager with orders and clinical information.
• Process documents via fax and manage supporting documentation.
• Coordinate onboarding with Marketing for referrals and patient service.
• Enter new patient data into the system and assist RN with reporting.
• Occasional on-call shifts may be required.
• Perform additional duties as assigned.

Intake Coordinator - LPN Home Health

Now hiring an LPN Intake Coordinator! This is a full-time office-based position with excellent benefits based at our corporate headquarters in Little Rock, AR. This position reports to our Intake Manager. This is a non-patient care office role requiring strong attention to detail and excellent computer & communication skills.

Duties & Responsibilities:

• Taking calls from prospective home health patients & referral sources
• Describing Care IV's home health service options to referral sources & patients
• Over the phone, evaluating & determining if patient is appropriate for home health care
• Verifying their payer source is in network with Care IV
• Assisting the Intake Manager with orders & other clinical informational assistance
• Sending & receiving faxes with necessary supporting documentation, signed orders, etc
• Follow up with physicians, clinics, long-term care facilities, hospitals, etc.
• Coordinating new patient onboarding with our Marketing Team to ensure satisfactory customer service to the referral sources & our patients
• Inputting new patient data into the computer system & assisting the RN with tracking & data reporting
• Some rotating on-call may eventually be required
• Additional duties as assigned

Required experience:

• We are seeking Licensed Practical Nurses whose license is active and in good standing with the Arkansas State Board of Nursing
• Recent experience working in a clinic or other professional medical office setting
• Excellent computer skills and a working knowledge of office machinery (computers, multi-line phones, fax machines, email, etc.)
• Solid clinical nursing knowledge with previous skilled nursing experience
• Experience with Home Health preferred, but not necessarily required
