The position requires an LPN license, a valid state driver’s license, and prior home health experience. A strong emphasis on coordination, communication, and attention to detail is essential for success.

Qualifications

• Must be a graduate of an accredited school of practical/vocational nursing.
• Current state LPN license.
• Valid driver’s license in the state.
• One year of experience as an LPN, home health preferred.
• Proof of hepatitis profile.
• Sales/marketing experience a plus.

Responsabilités

• Facilitate the intake referral process.
• Coordinate care with interdisciplinary team and referral source.
• Process all aspects of referral intake via Home Care Home Base software.
