Job summary

Bradford Hills Nursing & Rehabilitation Center is seeking an experienced Licensed Practical Nurse (LPN) to join our caregiving team in Bradford County, Pennsylvania. You will provide compassionate direct patient care, monitor residents, administer medications, and help with daily activities in a supportive setting.

As part of an interdisciplinary team, you will develop individualized care plans, document treatments accurately, and report changes to supervising nurses.

Qualifications

• Current LPN license in good standing.
• Experience delivering direct patient care in a long-term care setting.
• Ability to develop individualized care plans with a team.
• Strong documentation and record-keeping skills.

Responsibilities

• Provide direct patient care including assessments, medications, treatments, and procedures.
• Develop and implement individualized care plans with the interdisciplinary team.
• Maintain accurate records of treatments, medications, and patient progress.
• Report concerns to the nurse or supervisor promptly.
• Participate in quality improvement initiatives to improve patient outcomes.
• Coordinate with other departments to ensure seamless care transitions.
