Job summary

Bradford Hills Nursing & Rehabilitation Center is seeking a Licensed Practical Nurse (LPN) for a full-time position in Pennsylvania. The role focuses on direct patient care, including vital signs, wound care, and medication administration, within a collaborative interdisciplinary team.

Responsibilities include assessments, care plan development, medication administration, and accurate documentation to support quality patient outcomes in a fast-paced setting.

Qualifications

• Current LPN license in Pennsylvania.
• BLS certification required or equivalent.
• Ability to work in a fast-paced environment.
• Strong communication and interpersonal skills.
• Ability to lift, push and pull patients as needed.

Responsibilities

• Provide direct patient care, including vital signs, wound care, and medication administration.
• Conduct initial and ongoing assessments of patients' physical and psychological needs.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team.
• Administer medications and treatments as prescribed by physicians.
• Document patient information accurately and thoroughly in accordance with facility policies.
• Collaborate with other healthcare professionals to ensure seamless and efficient care delivery.
• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
