Eden Vista Rock Run, part of Vista Senior Living Management, seeks a Licensed Practical Nurse (LPN) to join our team and provide exceptional care to residents in a long-term care setting. You will assess health needs, administer medications, and collaborate with a multidisciplinary team to promote wellness and safety.

Qualifications

• Graduate or current student from an accredited nursing program.
• Hold a current Licensed Practical Nurse license.
• Minimum 1 year of LPN experience in a long-term care setting.
• Effective verbal and written communication skills.
• Commitment to interacting professionally and with care toward residents, families, and other staff.

Responsibilities

• Conduct health assessments and create personalized care plans for each resident.
• Administer medications and treatments according to physician orders and established protocols.
• Monitor residents' vital signs and address health status changes.
• Collaborate with interdisciplinary teams to provide holistic, person-centered care.
• Assist in developing and implementing wellness programs and activities.
• Educate residents and families on health topics and preventive care.
• Respond to medical emergencies and provide appropriate interventions.
• Maintain accurate resident medical records.
• Foster a supportive and compassionate environment for residents and families.
• Participate in ongoing training and professional development.
