This position is for an in-home care nurse, with a 1:1 nurse to patient ratio, caring for individuals who are intellectually or developmentally disabled. Specific location, hours, and days needed are detailed in the job title.

Primary Responsibilities:

• Manage healthcare plans for assigned patients according to physician’s instructions
• Provide personalized care including bathing, grooming, and assistance with daily activities
• Monitor vital signs, including temperature, blood pressure, and pulse
• Administer prescribed medications and maintain accurate records
• Check for signs of infection, bedsores, and wounds, providing appropriate care
• Educate and advise caregivers on patient care techniques
• Collaborate with healthcare team to develop and improve patient care plans
• Provide palliative care as needed to ensure patient comfort and well-being
