Chatham Hills Subacute Care Center is actively seeking a Licensed Practical Nurse (LPN) for the day shift to join our skilled nursing team. The role involves providing care for residents in the Skilled Unit (SNF) and Long Term Care Unit, including medication administration (IVs), vital signs, and ongoing treatments with outcomes monitoring.

The LPN will educate residents and families and collaborate closely with physicians, NPs, and other health care professionals.

Qualifications

• Current LPN license in good standing.

Responsibilities

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit.
• Medication administration, including IVs.
• Taking and recording vital signs.
• Providing resident care and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
• Supervising CNAs.
