Chatham Hills Subacute Care Center is seeking a Licensed Practical Nurse (LPN) for day shift to provide compassionate care in our skilled nursing and long-term care units. Responsibilities include administering medications, including IVs, taking vital signs, and coordinating with physicians and CNAs.

• Current license to practice as an LPN.

Responsibilities

• Provide care for residents of the Skilled Unit (SNF) and Long Term Care Unit.
• Administer medications, including IVs.
• Take and record vital signs.
• Provide resident care and monitor outcomes.
• Provide resident and family education.
• Communicate with physicians, NPs, families and other health care professionals.
• Supervise CNAs.
