Job summary

Signature HealthCARE is seeking a qualified LPN/LVN to join the Hart County Rehab & Wellness Center team in Kentucky. You will provide direct nursing care, handle medications, and support residents with daily activities within a patient-centered environment.

Ideal candidates have at least one year of clinical nursing experience, a supervisory background, and familiarity with electronic health records. CPR/BLS is preferred, with strong documentation and communication skills essential.

Qualifications

• Active LPN/LVN license in the state of practice required.
• Minimum 1 year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Strong observation, medication administration, and documentation skills required.
• Knowledge of Federal and State long-term care regulations.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs ensuring comfort, dignity, and safety.
• Supervise nursing assistants and medication aides during shifts.
• Monitor residents' condition and notify physician when changes occur.
• Develop and review individualized care plans with the interdisciplinary team.
• Complete accurate clinical documentation per facility policy.
• Coordinate admissions, transfers, and discharges.
• Participate in quality assurance activities and staff education.
• Perform other duties as assigned by the Director of Nursing.
