Responsibilities include coordinating with Direct Support Professionals, ensuring regulatory compliance, and participating in support plan meetings to honor residents’ choices and independence.

Qualifikationen

• Active and unencumbered LPN license.
• Current BLS / CPR certification.
• Experience with IDD, autism, or long-term care preferred.

Aufgaben

• Medication Administration: securely manage, order, and administer routine and psychotropic medications with adherence tracking.
• Health and Behavioral Monitoring: observe for health/behavior changes; follow up on medical appointments and treatments.
• Seizure and Feeding Management: monitor residents with seizure protocols, G-tube feedings, or modified diets.
• Direct Support Collaboration: work with DSPs to ensure ICPs are followed during daily routines.
• Regulatory Compliance: document in line with APD or similar regulations for logs and reports.
• Resident Advocacy: accompany residents to medical appointments to communicate health histories.
• Support Plan Meetings: participate in meetings with person-centered planning.
