Job Title: LPN I – Medication Assisted Recovery

Reports to: RN Program Manager – Substance Use

Location: Main Building, 151 N. Main St., Decatur, IL 62526

Schedule: Varied hours between 6a-4:30p Monday – Saturday; based on MAR program and clinic needs

FLSA Status: Non-Exempt

Position Type: Full-Time

Work Setting: In-Person

Position Overview

In keeping with the Mission, Vision, and Corporate Values of Heritage Behavioral Health Center, the LPN – Medication Assisted Recovery joins the Heritage nursing team to provide support and medication services to assist adults with a diagnosis of substance use disorder who are enrolled in the agency’s Medication Assisted Recovery (MAR) program. Services are provided at Heritage’s downtown office in Decatur, Illinois.

This position reports to the RN Program Manager – Substance Use and works collaboratively with clients, the nursing team, physicians, providers, and other members of Heritage teams to support each individual’s recovery.

This position involves regular direct contact with individuals served by Heritage, including adults living with substance use and mental health conditions. A compassionate, professional, trauma-informed approach is essential.

  • Represent Heritage’s mission and values through professional, respectful conduct at all times
  • Approach all interactions with clients using trauma-informed, recovery-oriented practices that are sensitive, empathetic, and effective
  • Work collaboratively with clients, the nursing team, physicians, providers, and other Heritage teams
  • Communicate openly and proactively with the RN Program Manager regarding client needs, concerns, and program operations
  • Maintain client confidentiality in accordance with HIPAA and 42 CFR Part 2
  • Demonstrate flexibility and a team-oriented mindset in the midst of ambiguity, uncertainty, and change
  • Follow all Heritage policies, procedures, and safety standards
Core Responsibilities
Medication Administration & Nursing Care
  • Provide daily medication administration in the Medication Assisted Recovery program
  • Provide nursing assessments, medication administration, medication monitoring, and medication training to assist clients with learning how to self-administer medications, monitor response to medications on target symptoms, properly secure and store medications, and communicate concerns to providers; support referral and care coordination
  • Accurately document in the electronic health record daily on interventions, the client’s response, and the client’s progress in treatment
Care Coordination & Client Education
  • Coordinate holistic care for individuals receiving medication assisted recovery, including primary care, labs, mental health or substance use treatment, and other identified needs/care
  • Provide education on health and wellness, including, but not limited to, chronic health disease self-management, infectious diseases, and primary care
  • Participate in a team-based approach to providing treatment, including collaboration with clinicians, nursing, healthcare providers, and other partners invested in the individual’s recovery
  • Attend clinical staffing(s) to collaborate with providers to coordinate treatment, including counselors, engagement staff, nursing, physician(s), and clinical leaders
  • Adhere to agency policies and procedures, accrediting body requirements for the Opioid Treatment Program (CARF), and federal and state guidelines (Illinois Division of Substance Use Prevention and Recovery, DEA, and SAMHSA)
  • Engage in ongoing learning of current best practices in behavioral health treatment
  • Perform other duties as assigned consistent with this role

The LPN – Medication Assisted Recovery will be successful in this role when:

  • Clients receive safe, accurate, and timely medication administration and monitoring
  • Documentation in the electronic health record is complete, accurate, and entered daily
  • Clients feel supported, respected, and treated with dignity throughout their recovery
  • Care is well-coordinated across primary care, labs, and behavioral health services
  • The nursing team, providers, and program leadership view this position as a reliable, collaborative teammate
  • The program maintains compliance with CARF, DEA, SAMHSA, and state requirements
Knowledge, Skills and Abilities
  • Familiarity with addiction/recovery and behavioral health
  • Thorough understanding of trauma-informed care practices, essential for providing optimal support and services to individuals in a manner that is sensitive, empathetic, and effective
  • Ability to work as a team player
  • Ability to tolerate ambiguity, uncertainty, and change
  • Ability to navigate electronic health records
  • Demonstrated above-average computer skills; proficient in Microsoft Word and Excel
  • Strong attention to detail and accuracy in medication handling and documentation
  • Ability to maintain client confidentiality in accordance with HIPAA and 42 CFR Part 2
Education and Experience
Required
  • Minimum 21 years of age
  • An active, undisciplined State of Illinois LPN licensure
  • A valid driver’s license, reliable means of transportation, and proof of automobile insurance coverage
  • Ability to obtain and maintain current CPR certification
Preferred
  • Experience in substance use treatment, behavioral health, or medication assisted recovery settings
  • Experience with medication administration in an outpatient or Opioid Treatment Program setting

Note: Equivalent combination of education and experience will be considered.

Physical Requirements and Working Conditions
  • Ability to stand, walk, and remain on feet for extended periods during medication administration
  • Ability to sit, stand, and work at a computer for periods of time
  • Ability to respond calmly and effectively in crisis or emergency situations
  • Regular direct contact with individuals served by Heritage, including those experiencing substance use and mental health challenges
  • May require travel between Heritage facilities and community locations
  • Must adhere to all Heritage policies, procedures, and safety standards
