Job summary

Hospice of the Valley in the Phoenix metropolitan area seeks a compassionate home health nurse to educate patients and families, manage pain and symptoms, and provide solace during difficult times. Care delivery occurs primarily in the patient's home or in facilities where care is provided.

You will work as part of an interdisciplinary team, building therapeutic relationships, documenting care, and supporting patients and families through challenging moments with professionalism and empathy.

Position Profile

The hospice home care nurse educates home care patients and families about the disease process, help manage and control pain and other symptoms and offer solace to those in distress. They are part of the interdisciplinary inpatient hospice team that supports people at critical junctures in their lives, making a difference in their quality of life. Care delivery primarily takes place in the patient's home or in a facility where the patient receives care.
