Job summary

HHCP is seeking a compassionate Registered Nurse to provide specialized hospice care, guiding patients and families with tailored plans of care and expert symptom management. As Case Manager, you will coordinate services, educate families, and collaborate with physicians to ensure comfort and quality of life in the home setting.

You will join a team focused on geriatrics, home health, hospice and palliative medicine, with a strong emphasis on advocacy, coordination, and compassionate care

Qualifications

• RN licensure in the state with active CPR certification.
• Bachelor’s degree and 1+ year hospice experience preferred.
• 1–2 years of acute care experience in an institutional setting, plus 1–2 years of home care.
• Strong observation, problem-solving, and communication skills to advocate for patients.
• Ability to lift, position, and transfer patients; prolonged standing, walking, bending.

What You Will Do

• Conduct comprehensive, ongoing assessments of patients to determine their physical, emotional, and hospice-specific needs.
• Serve as the dedicated Case Manager, designing, initiating, and regularly revising personalized plans of care.
• Provide high-quality, professional nursing care with a strong focus on effective pain and symptom management for optimal comfort.
• Empower and counsel patients and their families, providing essential health education, emotional support, and guidance on activities of daily living.
• Act as a vital link in the patient's care network by communicating changes in condition to physicians, obtaining orders, and collaborating in interdisciplinary group meetings.
• Supervise and support hospice aides to ensure the highest standard of care is delivered.
• Participate in our on-call rotation to ensure our patients always have the urgent support they need.
• Coordinate necessary medical equipment and services for the patient's home environment.

What You Bring

• Current RN licensure in the state and active CPR certification.
• Graduation from an accredited school of nursing (Bachelor’s degree and 1+ year of hospice experience are highly preferred).
• 1 to 2 years of recent acute care experience in an institutional setting, alongside 1 to 2 years of recent home care experience.
• Exceptional observation, problem-solving, and communication skills to effectively advocate for your patients.
• The physical ability to safely lift, position, and transfer patients, as well as handle supplies (requires prolonged standing, walking, bending, and stooping).
• Required visual and hearing acuity to perform clinical nursing assessments.
• A valid driver’s license and a reliable, insured vehicle in good working order to travel to our patients.
