Job summary

Transitions Hospice is seeking an experienced Night Nurse for overnight coverage in Illinois. The role supports the interdisciplinary care team from 8 PM to 8 AM with a rotating 4-day/3-day schedule including weekends.

You will assess, treat, and communicate patient needs with clinical confidence and compassion. Ideal candidates hold an active LPN license in the state, have nursing school accreditation, and possess hospice/home health experience.

Qualifications

• Active LPN license in the state of employment.
• Graduate of an accredited school of Nursing.
• Clinical experience in hospice, home health, or a related setting preferred.
• Comfort and composure working independently during overnight hours.
• Strong assessment skills and sound clinical judgment in high-pressure moments.
• Excellent documentation habits and communication skills.
• Valid driver's license and willingness to travel locally.

Responsibilities

• Delivering professional nursing care across the full nursing process within your scope of practice.
• Conducting thorough patient and family assessments and contributing to individualized care plans.
• Completing all visit types as needed to meet overnight patient and family needs.
• Administering medications and treatments per physician orders.
• Identifying nursing diagnoses from health assessment data and responding with sound clinical judgment.
• Recording accurate, timely clinical notes including pain and symptom management outcomes.
• Communicating changes in patient condition to physicians and obtaining orders as required.
• Coordinating with community health partners to support continuity of care.
• Ensuring equipment and necessary services are in place for patients.
• Maintaining and growing your clinical skills through continuing education and in-service participation.

Coverage Territory: Bloomington, Pontiac, Streator, Seneca, Ottawa, LaSalle Peru, Dwight, Marseilles, and surrounding areas.

Schedule: 8 PM–8 AM, alternating 4-day and 3-day weeks including weekends in rotation

Salary Range: $28.00 - $31.00 an hour
