Job summary

Amedisys, one of the largest home health and hospice companies in the U.S., is looking for a licensed practical nurse to join our full-time day shifts in patient homes. You’ll work with RNs and physicians to ensure high-quality, compassionate care in the comfort of patients’ homes.

Qualifications

• One year of clinical LPN experience.
• Current LPN license for the state(s) assigned.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met; notify them promptly when patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
