Maxim Healthcare is seeking compassionate Licensed Practical Nurses (LPNs) to provide skilled nursing care to adult and pediatric patients in their homes throughout Fond du Lac, WI. Join a team that values patient-centered care and offers flexible scheduling with competitive pay.

Qualifications

• Current and unrestricted LPN license in Wisconsin.
• Current BLS/CPR certification.
• TB test or Chest X-Ray.
• Previous nursing experience preferred (1 year or more).
• Strong clinical, communication, and documentation skills.
• Commitment to providing compassionate, patient-centered care.

Responsibilities

• Deliver skilled nursing care per physician orders and care plans.
• Assess, monitor, and document patients' health status.
• Identify changes in patient conditions and communicate findings.
• Develop and implement individualized care interventions.
• Collaborate with patients, families, and healthcare teams to ensure quality care.
• Maintain accurate and timely clinical documentation.
