Job summary

Maxim Healthcare is seeking compassionate and dedicated LPNs to provide high-quality care for adult patients in their homes throughout the Coon Rapids area. Flexible full-time and part-time schedules with competitive pay.

You'll deliver skilled nursing care, monitor health, document findings, and collaborate with families and healthcare teams to ensure safe, patient-centered care in home settings.

Qualifications

• Current and unrestricted LPN license in Wisconsin.
• Current BLS/CPR certification.
• Current TB test or Chest X-Ray.
• Previous nursing experience preferred (1 year or more).
• Strong clinical, communication, and documentation skills.
• Commitment to providing compassionate, patient-centered care.

Responsibilities

• Deliver skilled nursing care in accordance with physician orders and care plans.
• Assess, monitor, and document patients' physical and emotional health status.
• Identify changes in patient conditions and communicate findings appropriately.
• Develop and implement individualized care interventions.
• Collaborate with patients, families, and healthcare teams to ensure quality care.
• Maintain accurate and timely clinical documentation.
