Job summary

Assured Home Health, a part of the UnitedHealth Group family, is seeking a Licensed Practical Nurse to provide in-home patient care across Washington. You will deliver high quality nursing services per individualized care plans, document visits, and collaborate with care teams to ensure timely interventions and patient safety.

Applicants should have an active WA LPN license, current CPR or be willing to obtain within 90 days, and a valid driver’s license with reliable transportation for home

Qualifications

• Current and unrestricted LPN licensure in Washington state.
• Current CPR certification or ability to complete within 90 days of hire.
• Valid driver's license and access to reliable transportation.

Responsibilities

• Provides high quality clinical services within the scope of practice per plan of care and infection control procedures.
• Documents patient visits thoroughly in the electronic medical record as required.
• Participates in care coordination, communicates patient needs and changes in condition to the team.
• Function effectively in home settings regardless of patient age, race, or condition.
