Job summary

UnitedHealth Group is hiring a Licensed Practical Nurse in Home Health serving Washington state patients, via Assured Home Health, part of LHC Group within the Optum family.

As a field-based nurse, you will provide high-quality clinical care at patients' homes, document visits, and collaborate with a multidisciplinary team to coordinate care.

This role requires current WA LPN licensure, a valid driver’s license, and the ability to work flexible hours to meet patient needs.

Qualifications

• Current and unrestricted WA LPN licensure.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Provides high quality clinical services within the scope of practice in patients' homes.
• Documents patient visits thoroughly and completely in the EMR.
• Acts in care coordination with the health care team and communicates with patient, family, and providers.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
