Job summary

UnitedHealth Group, through Assured Home Health (LHC Group), seeks a Licensed Practical Nurse in Home Health to deliver quality patient care in patients' homes across Washington state. You will document visits, coordinate care, and collaborate with a multidisciplinary team to ensure compliant nursing services in a home health setting.

Requires Washington LPN licensure, a valid driver's license or reliable transportation, and the ability to manage multiple tasks with strong communication.

Qualifications

Current and unrestricted LPN licensure in state of Washington.
Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
Ability to manage multiple tasks simultaneously.
Excellent communication and organizational skills.
Ability to work flexible hours to meet client needs.

Responsibilities

Provides high quality clinical services within the scope of practice, per plan of care and infection control procedures.
Documents patient visits thoroughly in the electronic medical record and EMR systems.
