Job summary

Penn Home Health Care is seeking compassionate Licensed Practical Nurses (LPNs) to provide skilled nursing services to patients in their homes under the supervision of a Registered Nurse and in accordance with physician orders. Service areas include Philadelphia, Delaware, Chester, Montgomery, Bucks, and Berks Counties in Pennsylvania.

This Per Visit / PRN role offers flexible scheduling and competitive per-visit compensation, with growth opportunities within a Medicare-certified home health

Qualifications

• Current Pennsylvania LPN license in good standing.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• One year of clinical nursing experience preferred.
• Home health experience preferred but not required.

• Provide skilled nursing visits in accordance with the patient's plan of care.
• Monitor patient status and report significant changes to the supervising RN and physician.
• Provide medication education and reinforcement of nursing teaching.
• Assist with disease management and chronic condition monitoring.

• Document patient care accurately and timely.
