Job summary

Elara Caring in Ohio seeks a Licensed Vocational / Practical Nurse to deliver high-quality home-based care. You will follow patient care plans, monitor progress, and educate patients and families in self-care at home, coordinating with the RN Case Manager for updates.

The role requires an LVN/LPN license, 1 year of clinical care experience, and ability to travel between patient homes. Hospice/home health experience is preferred and a driving route is necessary.

Qualifications

• Current State License as LVN/LPN.
• Graduate of an accredited nursing program (Certificate, Diploma, or Associate).
• 1 year of clinical care experience.
• Ability to frequently lift up to 50 pounds and assist transfers.
• Valid driver’s license or reliable transportation for home visits.
• Hospice or home health experience is preferred.

Responsibilities

• Follow each patient’s individualized care plan and physician orders.
• Monitor patient progress and report changes to the care team.
• Help patients and caregivers learn self-care techniques to support health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles per agency guidelines.

• Help prevent avoidable hospitalizations through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
