Job summary

Health Watch Health Care in Altus, OK is seeking a compassionate Licensed Practical Nurse (LPN) for full-time home health care. You will deliver one-on-one skilled nursing care in patients' homes, administer medications, observe conditions, and document care plans under supervision.

You'll build strong relationships with patients and families while maintaining independence and collaborating with a clinical team to ensure safe, quality outcomes at home.

Qualifications

• Active LPN/LVN license (state of practice).
• Experience in home health, hospice, long-term care, or clinical nursing preferred.
• Strong clinical, communication, and organizational skills.
• Ability to work independently while managing multiple patients.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide skilled nursing care under the direction of a Registered Nurse and physician.
• Administer medications and treatments as ordered.
• Monitor patient conditions and report changes to the supervising RN.
• Reinforce patient and caregiver education on medications, treatments, and disease management.
• Assist with developing and following individualized plans of care.
• Complete timely and accurate clinical documentation.
• Collaborate with physicians, caregivers, and the interdisciplinary care team.
