Mays House Call Home Health is seeking a compassionate Licensed Practical Nurse (LPN) to provide skilled, one-on-one care in patients' homes. This full-time role emphasizes patient safety, independence, and high-quality clinical outcomes in a home health setting.

You will work under the direction of an RN and physician, manage medications and treatments, monitor patient conditions, and reinforce education for patients and caregivers.

Responsabilités

• Provide skilled nursing care under the direction of a Registered Nurse and physician.
• Administer medications and treatments as ordered.
• Monitor patient conditions and report changes to the supervising RN.
• Reinforce patient and caregiver education on medications, treatments, and disease management.
• Assist with developing and following individualized plans of care.
• Complete timely and accurate clinical documentation.
• Collaborate with physicians, caregivers, and the interdisciplinary care team.
