Job summary

Angels Care Home Health is seeking a Licensed Practical Nurse (LPN) to administer skilled nursing care under the supervision of a registered nurse for clients in their homes in Altus, OK. The LPN coordinates care with the interdisciplinary team, patient and family, and referring agency.

The ideal candidate will be a graduate of an accredited LPN program with an active state license, a valid driver's license, CPR and Hepatitis profile, and at least two years of LPN experience, preferably in home

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state LPN license.
• Driver's license.
• Two years’ LPN experience, home health preferred.
• CPR and Hepatitis profile.

Responsibilities

• Administer skilled nursing care under the supervision of a registered nurse.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
• Provide care in clients' place of residence (home health).
