Mass General Brigham Home Care, Inc. is seeking a Licensed Practical Nurse to provide compassionate home-based nursing care across the Boston area. You will work with the RN and interdisciplinary team, managing visits, documentation, and patient needs with a focus on independence and work-life balance.

A generous sign-on bonus of $5,000 (pro-rated for part-time) is offered to welcome new hires. Flexible scheduling includes full-time or part-time options with opportunities that minimize evenings

Formación

• Active Massachusetts LPN license.
• Minimum 1 year of acute or sub-acute nursing experience.
• Home care experience preferred.
• Strong clinical, communication, and organizational skills.
• Ability to work independently and adapt in dynamic environments.
• Comfortable with electronic documentation and mobile technology.
• Ability to travel within assigned territory.

Responsabilidades

• Provide nursing care and monitor patients' conditions and report changes to RN or provider.
• Assist with patient assessments, admissions, and care planning with RN.
• Implement plans of care and reinforce education for patients and caregivers.
• Administer medications and treatments as ordered within LPN scope.
• Coordinate care with patients, families, and healthcare providers.
• Maintain accurate clinical documentation and review patient charts.
