Job summary

Qualifications

• Graduate of a state approved practical nursing program.
• At least 1 year of clinical experience; home care preferred.
• Current LPN license to practice in Massachusetts.
• Valid driver's license with reliable transportation; fluent in English.

Responsibilities

• Provide direct nursing care to patients in the home per agency policy.
• Collaborate with the nurse case manager and refer families to agency resources.
