Job summary

Care Central VNA & Hospice is seeking an LPN to provide compassionate home health nursing in the Auburn/Worcester area. Part-time and full-time hours are available, with responsibilities including direct patient care under the nurse case manager and adherence to agency policies.

The LPN will assess patients, administer medications as ordered, and coordinate with families and community services to support rehabilitation and comfort in the home setting.

Qualifications

• Graduate of a state approved practical nursing program.
• At least 1 year of clinical experience; home care preferred.

Description

Part-time and Full-time hours available. The LPN will care for patients in the Auburn/Worcester Area

The LPN works under the guidance and oversight of the nurse case manager, to give direct nursing care to patients in the home within the policies of the Agency. The case manager is responsible for the LPN’s direction. The LPN provides nursing services, contributing to the treatment and rehabilitation of the patient. Consults with and refers families to appropriate personnel within the Agency or other community services as needed.

Requirements

• Education: Graduate of a State approved school of practical nursing.
• Experience: At least one year of clinical experience. One year of home care experience preferred.
• Licensure: Current license to practice as a practical nurse in the Commonwealth of Massachusetts.
