Job summary

Volunteers of America Mid-States is seeking an Licensed Practical Nurse (LPN) Home Coordinator in Middle Tennessee to provide nursing leadership for two IDD medical homes. You will supervise LPNs and DSP staff, coordinate healthcare services, manage medications, and ensure regulatory readiness to deliver person-centered supports with dignity and independence.

Qualifications

• Current, active, unrestricted Licensed Practical Nurse license in Tennessee.
• Experience leading residential teams supporting individuals with IDD.
• Knowledge of medication administration, health monitoring, and clinical documentation.
• Ability to manage two residential locations with clear communication and regulatory readiness.
• Commitment to person-centered supports, dignity, and community inclusion.

Responsibilities

• Provide nursing services within LPN scope and policy.
• Monitor health status and communicate significant changes to clinicians and guardians as needed.
• Coordinate routine and specialty healthcare, including hospital discharge and follow-up care.
• Ensure medication systems are implemented, documented, and reviewed for accuracy.
• Oversee daily operations of two IDD homes and maintain safe, welcoming environments.
• Supervise LPN and DSP staff, support onboarding, scheduling, and performance feedback.
• Maintain LPN competency and ensure staff are trained before procedures.
• Align services with individual-centered plans, rights, and community participation.
• Follow Tennessee DIDD, Medicaid, HCBS, licensing, HIPAA, and agency requirements.
