Addus HomeCare is seeking a Licensed Practical Nurse to perform home-based nursing assessments and care under RN supervision in Ashford, AL. The role emphasizes implementing the care plan and coordinating with the healthcare team.

The LPN will conduct nursing tasks in clients’ homes, monitor vital signs, document findings, and provide family education while ensuring confidentiality and adherence to regulations. A valid LVN/LPN license in Alabama is required.

Formación

• Graduate from an accredited school of practical nursing.
• Licensed as a vocational/practical nurse in the state of practice.

Responsabilidades

• Perform nursing procedures within LPN/LVN scope in clients' homes.
• Provide patient and family teaching per plan by RN.
• Monitor vital signs and document observations.
• Administer medications as instructed and document accurately.
• Coordinate care with patient, family and healthcare team.
• Maintain confidentiality and comply with HIPAA regulations.
